- Your contact information, including your address, telephone number, and email address;
- A statement by you that you have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law;

Repeat Infringers
In accordance with the DMCA and other applicable laws, we have adopted a policy of terminating, in appropriate circumstances, the accounts of users who are deemed to be repeat infringers.
A "repeat infringer" is a user who has been the subject of two (2) or more valid DMCA takedown notices. We maintain internal records of all DMCA notices received to identify repeat infringers.
Upon identification of a repeat infringer, we will:
- Review the user's account and history of alleged infringements
- Terminate the user's account if we determine, in our sole discretion, that termination is warranted
- Take reasonable steps to prevent the user from creating new accounts
We reserve the right to terminate any user's access to the Service if we believe, in our sole discretion, that the user has infringed the intellectual property rights of others, even if the user has not been the subject of two or more DMCA notices.
